Physicians may make appointments for their patients with any of our physicians by completing any one of the following:

1. Download our Neurosurgery Fast Fax Referral Form. Complete the form and fax to (352) 392-2443.
2. Call our offices in Gainesville at (352) 273-6990 (Option 1) OR (800) 346-3275 (Option 1).
3. Click here to email us, telling us your name, phone number and a good time to contact you.

Please print the Patient History Questionnaire for your patient ask him/her to bring this completed form to the appointment.
