Q: Do I need a referral from my doctor before I call?

A: We only need a referral if your insurance requires one for the visit. Our referral team will assist in obtaining insurance authorization for your visit, as well as requesting relevant medical records.

Q: Do you accept my insurance?

A: A list of accepted insurances is available here. Our referral team can help with insurance questions.

No Insurance? Just let us know when you call.

Q: What should I bring to my first appointment?

A: Here’s our first appointment checklist:

  • Your completed patient history questionnaire
  • All pertinent medical records
  • Your X-ray/CT/MRI scan on CD, not just reports
  • If you have a cardiac history, bring results of your last EKG and/or other tests
  • Your insurance card /ID

Q: I have a question or need to adjust my appointment time?

A: New Patients contact (352) 273-6990. Established patients contact the Neurosurgery Main Office at 352-273-9000 with questions or appointment requests.
