Q: Do I need a referral from my doctor before I call?
A: We only need a referral if your insurance requires one for the visit. Our referral team will assist in obtaining insurance authorization for your visit, as well as requesting relevant medical records.

A: A list of accepted insurances is available here. Our referral team can help with insurance questions.
No Insurance? Just let us know when you call.
Q: What should I bring to my first appointment?
A: Here’s our first appointment checklist:
- Your completed patient history questionnaire
- All pertinent medical records
- Your X-ray/CT/MRI scan on CD, not just reports
- If you have a cardiac history, bring results of your last EKG and/or other tests
- Your insurance card /ID
